One of the known methods to perform transfusion therapy to remove a considerable amount of leukocytes (up to 85%) and proteins from the original plasma (99%), is the washing (by manual procedures) with physiological saline solution (NaCl) at 0.9% of RBC units (red blood cells). This method is less expensive and less efficient than the use of blood filters, but with proper use, it always achieves its dual purpose in the removal of plasma, leukocytes and microaggregates. It should always be noted that this procedure does not protect the patient against alloimmunization with HLA antigens.

Uno de los métodos conocidos para ejecutar una terapia transfusional que permita retirar una cantidad considerable de leucocitos (hasta en 85%) y las proteínas del plasma original (en 99%), es el lavado (por procedimientos manuales) con solución salina fisiológica (NaCl) al 0,9% de unidades de GR (glóbulos rojos). Siendo un método menos costoso y menos eficiente a diferencia del uso de filtradores sanguíneos, pero siempre se consigue, con el debido uso, su doble propósito en la eliminación de plasma, leucocitos y microagregados. Teniendo siempre cuidado, que este procedimiento no protege al paciente contra la aloinmunización con los antígenos HLA.


The therapy is used to prevent anaphylactic reactions and in most cases, it is used when leukocyte-removing filters (which are more expensive) are not available.

Now, what does GRL mean?

La terapia se usa para prevenir las reacciones anafilácticas y en la mayoría de los casos, se usa cuando no se dispone de filtros removedores de leucocitos (que son de más alto costo).

The RBCs must undergo continuous washing with cold isotonic saline solution (3-4 °C) from 3 to 6 times, in order to remove mainly plasma proteins (approximately 98%), with a hematocrit at the end of the procedure between 70% and 80%, in a volume of approximately 180 ml. To perform the process, it is necessary to open the system, so the administration of the component should be done as soon as possible, being able to be stored in the refrigerator only for 24 hours after the end of the procedure, since opening the blood unit implies a great risk of contamination when the washing is performed.

Los GR deben someterse al lavado continuo con solución salina isotónica fría (3-4 °C) de 3 a 6 veces, con el fin de remover fundamentalmente las proteínas plasmáticas (aproximadamente un 98%), con un Hematocrito al final del procedimiento entre 70% y 80%, en un volumen aproximado de 180 ml. Para realizar el proceso, es necesario abrir el sistema, por lo que la administración del componente deberá hacerse tan pronto como sea posible, pudiendo almacenarse en nevera solo durante las 24 horas después de finalizado el mismo, ya que al abrir la unidad sanguínea implica un gran riesgo de contaminación cuando se realiza el lavado.

The formal indication for this component is to prevent anaphylactoid or anaphylactic post-transfusion reactions. They can be used in hyperkalemic patients and for the management of cases of hemolytic disease of the newborn in which the only transfusion option is the administration of RBCs from the mother.

La indicación formal de este componente es prevenir las reacciones postransfusionales anafilactoides o anafilácticas. Pueden ser utilizados en pacientes hiperkalémicos y para el manejo de los casos de enfermedad hemolítica del recién nacido en los cuales la única opción de transfusión es la administración de los GR de la madre.


In addition, physicians recommend this treatment to combat anemias in patients who have antibodies against human plasma proteins or derivatives thereof, especially anti-IgA. It is also used in patients who manifest severe symptoms of allergic reaction after receiving transfusions of this blood component. It is also used in patients undergoing extracorporeal surgery, to eliminate microaggregates that damage the microvasculature. In cardiopathic and nephropathic patients, or also when they suffer from paroxysmal nocturnal hemoglobinuria.

Además, los médicos recomiendan este tratamiento para combatir anemias en pacientes que poseen anticuerpos contra las proteínas del plasma humano o derivados de éste, especialmente anti-IgA. También en pacientes que manifiestan síntomas severos de reacción alérgica luego de recibir las transfusiones de este componente sanguíneo. Se utiliza también en pacientes de cirugía extracorpórea, para eliminar microagregados que lesionan la microvasculatura. En pacientes cardiópatas y nefrópatas, o también cuando padecen hemoglobinuria paroxística nocturna.

The dosage should be without variation in relation to the transfusion of unmodified RBCs. It is also fair to notify about contraindications and any precautions to be taken due to the potential for bacterial contamination when performing the washing procedure, only to be carried out when there is a formal indication.

La dosificación debe ser sin variación con relación a la transfusión de GR no modificados. También es justo notificar acerca de las contraindicaciones y todo tipo de precaución que se debe tener debido al potencial de contaminación bacteriana al realizar el procedimiento de lavado, sólo se llevará a cabo cuando exista indicación formal.
