Anyone who follows me through the texts here or even through "real" life knows that I've been on a journey of healing my generalized anxiety disorder for years. I've even posted here a lot about this journey, in addition to talking about some of the medicines I've used to try to control and alleviate this problem. Whenever I start a new stage of treatment I have good expectations of improvement and I finally feel free from suffering, so it was in the last and most recent "chapter" of this journey, when I started using an SNRI, Duloxetine. I wanted to give modern antidepressants a "last chance" before giving up on them and turning to more rudimentary, or perhaps more alternative, systems of treatment. Duloxetine has a similar mechanism to Venlafaxine and, in addition to serotonin, it has a good Noradrenaline reuptake inhibition ratio, which would make it less zombifying than traditional SSRIs (such as Sertraline, Escitalopram, Fluoxetine, etc). Anyone who has used these selective inhibitors knows what I'm talking about! You can even have a great answer in terms of improving depression and anxiety but you trade them for a kind of robotization, flattening the emotional lines and feeling of being just a zombie living experiences without actually experiencing them. In the case of Duloxetine, that wasn't even my complaint, my problem was much more dense than just physical symptoms (which she also had, such as: jaw tension, eyelid contraction and genital insensitivity), the problem was that it seems that the mechanism of The substance's action was only able to contain the acute anxiety and panic (which were my biggest complaint in fact), but somehow made room for an already existing depression that couldn't be overcome by chemical therapy or worse, was CAUSING this depression .

It makes more sense to assume that these depressive feelings were already mine, I prefer not to believe that an antidepressant triggers such severe depression out of nowhere (yes, those were hard days). In my mind's eye I imagine that the drug managed to just hit the top of the iceberg and end the "flight or fight" state that was completely dysregulated and on red alert in me. I spent my days in a state of anxiety and always ended up in a panic attack that I could only get around with a benzodiazepine. As Duloxetine had relieved this acute crisis, there was room to look deeper at oneself, and then I found this great iceberg plunged into the sea of ​​consciousness, where depression took on a much more extensive and unobservable space that permeated almost all sectors of my life, and when faced with it, all I could do was feel it, as an automatic response. I insisted on a few months of treatment, tortuous months of waiting for the improvement that didn't come, and then I used that time to study more and more about what seemed to be the only possible solution since everything that was traditional and more "up to date" had failed. Little did I imagine that I would delve into such an unexplored and complex world.

It had been a while since I heard about a researcher named Ken Gillman who had been causing a revolution in the conception of monoamine oxidase inhibitors (or iMAOs), demystifying 90% of what was said about them until then, and which they continue to claim without sufficient knowledge. Dr Ken Gillman has dedicated all of his current years to researching and publishing content about MAOIs, breaking down prejudices and mythologies about what are the oldest antidepressants in our society. Often you will read that they are an old-fashioned, dirty and class C class of drugs, but the curious thing is to discover that none of this is because their mechanisms are not efficient or something, quite the contrary, they are by far the MOST EFFICIENT drugs and potent that we have news within the psychiatric classes until then. The work of the new substances that began with the advent of Fluoxetine is to go more and more against this and create more specific and filtered substances, which act less and less efficiently in psychiatric problems in a vertiginous search for action hyper focused on serotonin and forgetting to all other possible and necessary combinations between neurotransmitters! But why is iMAO not famous after all? Why isn't this type of medication out there being prescribed daily in countless psychiatric wards, out of sheer ignorance, and I can prove it!

The main fear factor of prescribing an iMAO drug (like Tranylcypromine, which is the drug I'm using) comes from a tradition of DATED and not up-to-date studies and articles on the substance's dangers in relation to two elements: diet and drug interactions. And here I am, based on great up-to-date studies and lucid reviews, to demystify both. I will start by talking about the most controversial topic, which is the relationship between MAOIs (especially irreversible ones) and TYRAMINE. The MAOIs started their journey badly because of adverse reactions of the substance with some foods with tyramine, historically mainly cheese! Yes, there have been some reported cases of death from cheese consumption in people treated with monoamine oxidative inhibitors! But nowadays it is known that not only is this type of cheese rich in high concentrations of tyramine extremely rare, but probably very expensive. The impression you get when you talk to a psychiatrist or neurologist (or anyone else) unaware and not up to date on the subject is that you will have to go on a very strict diet to avoid your own death and that a slip will take you to the ICU, and this is a MYTH! It is now known that very few foods in general contain high levels of tyramine and that few basic rules already prevent you from any problems, and that if a tyramine reaction occurs in your body, your pressure will not kill you, you can have it. a passing hypertensive crisis based on how much food you ate and this will pass and you will be fine, the problem is more related to the lack of information from professionals who can assist you if you go to the emergency room with a hypertensive crisis and tell them who is intoxicated with tyramine, some actions can be more catastrophic than the perceived risk itself, such as trying to lower your blood pressure too fast. From my own experience of the last month on Tranylcypromine (Parnate) and eating EVERYTHING I ever ate, I can say that 90% of food is not high in tyramine, maybe all I've been doing is avoiding foods stored for too long and consuming certain things that they're not even important to me! Oh, and yes, I EAT CHEESE, EVERY DAY!

The other problematic issue is about Drug Interactions, and these can have disastrous consequences for you, if you unknowingly use some substances, but what does this problem consist of? Unlike what was thought of in the problem, there are not so many interactions, they are mainly and more seriously focused on the use of drugs with serotonergic relationships, commonly SSRIs and variants! Some of the few but critical (even fatal) reported cases of serotonin syndrome occurred precisely because unsuspecting professionals induced dangerous combinations of substances in patients using MAOI. This causes the body/brain to be flooded with serotonin and triggers a fatal crisis. But just don't mix, it's not like not being able to take a headache medication! Of course, there are other dangerous combinations, but much less "traditional", such as the careless use of standard doses of stimulants like Aderall or the indiscriminate use of cocaine, in addition to the fatal mixture with MDMA (do not use MDMA with iMAO under any circumstances).

Now, speaking a little of my personal experience in order to finish this text once and for all. First of course it is necessary to be lucid and explain that it is not all a bed of roses, just like any substance, the use of Tranylcypromine has its disadvantages but NOTHING comes close to the disadvantages I have had with SSRIs. I don't have any sexual side effects (that alone beats ALL common antidepressants, because ALL cause this), I don't have any zombification or robotization effects, and nothing that is so conventional in other drugs. However, I'm still adapting to the substance and I had some small changes in blood pressure at the time of taking it (and not with food), but nothing significant and that can be easily reversed with a beta-blocker. In addition, the intense stimulation of Tranylcypromine (which has a very similar structure to amphetamine) has made me a little irritated, but very, very productive, focused and dedicated to anything and everything I need to do. I'm still at the beginning of my treatment and I can't say much more than that, since I started I haven't had more intense depressive episodes like I've had before and not even a minute of anxiety or panic, they seem like things that are in the past to never come back . I still need to see if in the long term I will adapt to the medication, but everything indicates that I will. I just need to learn to overcome a certain limitation with medications and interactions, avoid certain drugs and very rare foods. My sanity would be worth more than that, even. Because when I started I was willing to give up much more, based on false food lists and unrealistic medical warnings without knowledge. I wonder how many people could be benefiting from the substance and just go through years of treatment in vain until some doctor tired of trying something will suggest an iMAO for them. I hope that changes!

Quem me acompanha pelos textos aqui ou mesmo pela vida "real" sabe que estou em uma jornada de cura de meu transtorno de ansiedade generalizado há anos. Inclusive já postei aqui bastante sobre essa jornada, além de falar de alguns dos medicamentos que já fiz uso para tentar controlar e amenizar esse problema. Sempre quando começo uma nova etapa de tratamento eu tenho boas expectativas de melhora e sinto-me enfim livre de sofrimento, assim foi no último e mais recente "capítulo" dessa caminhada, quando iniciei o uso de um ISRSN, a Duloxetina. Eu queria dar uma "última" chance aos antidepressivos modernos antes de desistir deles e apelas para sistemas mais rudimentares de tratamento, ou mais alternativos talvez. A Duloxetina tem um mecanismo semelhante a Venlafaxina e além da serotonina tem uma boa relação de inibição de recaptação de Noradrenalina, o que a tornaria menos zumbificante que os tradicionais ISRS (como Sertralina, Escitalopram, Fluoxetina, etc). Quem já usou esses inibidores seletivos sabe bem do que eu estou falando! Você pode até ter uma ótima resposta em termos de melhora da depressão e ansiedade mas as troca por uma espécie de robotização, achatamento das linhas emocionais e sensação de ser apenas um zumbi vivendo experiências sem experimentá-las de fato. No caso da Duloxetina nem era essa minha reclamação, o meu problema foi muito mais denso que apenas sintomas físicos (que ela também tinha, tais quais: tensão mandibular, contração da pálpebra e insensibilidade genital), o problema foi que parece que o mecanismo de ação da substância só foi capaz de conter a ansiedade aguda e o pânico (que eram minha maior queixa de fato), mas de alguma forma abriram espaço para uma depressão já existente que não conseguia ser superada pela terapia química ou pior, estava CAUSANDO essa depressão.

Faz mais sentido supor que esses sentimentos depressivos já eram meus, prefiro não acreditar que um antidepressivo desencadeie do nada uma depressão tão severa (sim, foram dias difíceis). Em minha mente eu imagino que o medicamento conseguiu apenas atingir o topo do iceberg e acabar com o estado de "fuga ou luta" que estava completamente desregulado e em alerta vermelho em mim. Eu passava os dias em estado de ansiedade e sempre acabava em uma crise de pânico que só me restava contornar com um benzodiazepínico. Tendo então a Duloxetina aliviado essa crise aguda, abria-se espaço para olhar mais fundo para si mesmo, e encontrei então esse grande iceberg mergulhado no mar da consciência, onde a depressão tomava um espaço muito mais extenso e não observável que permeava quase todos os setores de minha vida, e ao deparar-se com isso só me restava senti-la, como resposta automática. Insisti alguns meses no tratamento, meses tortuosos de espera pela melhora que não veio e então eu usei esse tempo para estudar mais e mais sobre o que parecia ser a única solução possível já que tudo que havia de tradicional e mais "atual" havia falhado. Mal imaginava que iria mergulhar em um mundo tão inexplorado e complexo.

O trabalho das substâncias novas que se iniciou com o advento da Fluoxetina é ir cada vez mais na contramão disso e criar substâncias mais específicas e filtradas, que agem cada vez menos eficientemente nos problemas psiquiátricos numa busca vertiginosa pela ação hiper focadas na serotonina e esquecendo de todas as outras combinações possíveis e necessárias entre os neurotransmissores! Mas por que afinal o iMAO não é famoso? Por que esse tipo de medicamento não está por aí sendo prescrito diariamente nas inúmeras salas psiquiatricas, por pura ignorância, e eu posso provar!

O principal fator de medo da prescrição de um medicamento iMAO (como a Tranylcypromina, que é o medicamento que estou usando) vem de uma tradição de estudos e artigos DATADOS e não atualizados sobre os perigos da substância em relação a dois elementos: a alimentação e as interações medicamentosas. E cá estou eu para embasado em ótimos estudos atualizados e revisões lúcidas desmistificar ambos. Vou começar a falando do tema mais polêmico, que é a relação dos iMAOs (principalmente os irreversíveis) com a TIRAMINA. Os iMAOs começaram mal sua jornada exatamente por reações adversas da substância com alguns alimentos com tiramina, historicamente principalmente o queijo! Sim, houve alguns casos reportados de morte por consumo de queijo em pessoas que tratavam-se com inibidores de monoamina oxidade! Mas hoje em dia sabe-se que não só esse tipo de queijo rico em concentrações altas de tiramina são raríssimos como provavelmente caríssimos. A impressão que se dá quando conversa com um psiquiatra ou neurologista (ou qualquer pessoa) desavisada e não atualizada no tema é de que você terá que entrar em uma dieta rigorosíssima para evitar sua própria morte e que um deslize lhe levará para a UTI, e isso é um MITO! Atualmente sabe-se que pouquíssimos alimentos em geral contem altos níveis de tiramina e que poucas regras básicas já lhe previnem de qualquer problema, e que caso aconteça uma reação da tiramina em seu corpo, a sua pressão não irá lhe matar, você pode sim ter uma crise hipertensiva passageira baseada em quanto comeu do alimento e isso irá passar e você irá ficar bem, o problema está mais relacionado na falta de informação dos profissionais que podem lhe atender caso vá até o pronto-socorro com uma crise hipertensiva e diga-lhes que está intoxicado de tiramina, algumas ações podem ser mais catastróficas do que o suposto risco em si, como tentar baixar sua pressão rápido de mais. Por experiência própria do último mês medicado com Tranylcypromina (Parnate) e comendo TUDO que sempre comi, posso dizer que 90% dos alimentos não contém altos índices de tiramina, talvez tudo que tenho feito é evitar alimentos armazenados por tempo demais e consumir certas coisas que nem são importantes para mim! Ah, e sim, EU COMO QUEIJO, TODOS OS DIAS!

A outra temática problemática é à respeito das Interações Medicamentosas, e essas sim podem trazer consequências desastrosas para você, caso desavisadamente utilize algumas substâncias, mas no que consiste esse problema? Diferente do que era pensado no problema, não são tantas as interações, elas estão focadas principalmente e mais gravemente ao uso de medicamentos com relações serotoninérgicas, comumente os ISRS e variantes! Alguns dos poucos mas críticos casos relatados (fatais, inclusive) de síndrome serotoninérgica ocorreram exatamente por que desavisados profissionais induziram combinações perigosas de substâncias em pacientes com uso de iMAO. Isso leva o corpo/cérebro a ser inundado por serotonina e desencadeia uma crise fatal. Mas é só não misturar, não é como não poder tomar um medicamento de dor de cabeça! Claro, existem outras combinações perigosas, mas muito menos "tradicionais", como o uso descuidado de doses padrão de estimulantes como o Aderall ou o uso indiscriminado de cocaína, além da fatal mistura com MDMA (não use MDMA com iMAO em hipótese alguma).

Agora, falando um pouco de minha experiência pessoal para poder terminar esse texto duma vez. Primeiro é claro que é necessário ser lúcido e explicar que não é tudo um mar de rosas, assim como qualquer substância, o uso da Tranylcypromina tem suas desvantagens mas NADA chega perto das desvantagens que eu já tive com os ISRS. Não tenho efeitos colaterais sexuais (só isso já desbanca TODOS os antidepressivos comuns, por que TODOS causam isso), não tenho efeitos de zumbificação ou robotização, e nada do que é tão convencional nos outros medicamentos. Porém, ainda estou em adaptação da substância e tive algumas pequenas alterações da pressão sanguínea na hora da tomada (e não com a alimentação), porém nada significativo e que pode ser revertido com um beta-bloqueador tranquilamente. Além disso a estimulação meio intensa da Tranylcypromina (que tem uma estrutura bem semelhante à anfetamina) tem me deixado um pouco irritado, porém muito, muito produtivo, focado e dedicado em tudo e qualquer coisa que eu precise fazer. Ainda estou muito no começo de meu tratamento e não posso afirmar muito mais do que isso, desde que comecei não tive mais episódios depressivos intensos como vinha tendo anteriormente e nem mesmo um minuto de ansiedade nem pânico, parecem coisas que ficaram no passado para não voltar. Eu ainda preciso observar se à longo prazo irei me adaptar a medicação, mas tudo indica que sim. Só preciso aprender a superar certa limitação com medicações e interações, evitar certas drogas e raríssimos alimentos. Minha sanidade valeria mais do que isso, inclusive. Pois quando comecei eu estava disposto a abrir mão de muito mais, baseava-me nas falsas listas alimentares e irreais advertências médicas sem conhecimento. Fico pensando quanta gente poderia estar sendo beneficiada pela substância e simplesmente passa por anos de tratamento em vão até que algum médico cansado de tentar algo irá sugerir para ele um iMAO. Eu espero que isso mude!

Isso foi tão bom que salvei teu post para pesquisar mais sobre isso depois.

Já tomei sertralina e lembro muito bem como eu ficava. Estou na venlafaxina agora e não está sendo eficiente, nem um pouco. Minha ansiedade continua me afetando 24/7 com crises de pânico ou estresse acontecendo.
