NEUROBLASTOMA, One somewhat unknown and ignored, but that may become more common than we ourselves think. But, what is it?
Neuroblastomas are embryonal cancers of the sympathetic nervous system, that is to say that it is formed from the very moment of our development, when we are still fetuses developing in the maternal uterus, this type of tumors can vary among a wide variety, they can be from one with spontaneous cure even without the use of treatment, to extremely aggressive tumors that do not respond even to the most intense treatment, and certainly scientific advances are improving more and more therapeutic measures although many patients still succumb to aggressive forms despite all kinds of treatments.

What types of people may be affected?

It is the most common extracranial solid tumor in children, and the most frequently diagnosed cancer in infants, that is, the period from one month of life until the time they turn 3 years old. In the USA there are approximately 600 cases diagnosed each year, 90% of these are discovered before the age of 6 years, and it is slightly more frequent in males.

WHAT CAN LEAD ME TO DEVELOP CANCER?


The truth is that the cause is still unknown in most cases, however, a small number of cases are associated with certain genetic mutations, in addition to increasing the risk of maternal and even paternal chemical exposures.

In addition, this tumor has been associated with certain disorders, such as neurofibromatosis type 1, Hirschsprung's disease (also called aganglionosis), and certain other pathologies.

MANIFESTATIONS OF THE DISEASE

As I mentioned before, this tumor derives from the sympathetic nervous system (SNS), and can appear anywhere in this, the SNS is a portion of the nervous system that covers from the thoracic to the lumbar region, and can appear anywhere in this area, being the most frequent manifestation in the suprarenal glands, and the signs and symptoms that it will present will depend on the location and its extension. This type of tumor can metastasize through the blood (hematogenous), lymph (lymphatic) or by local invasion, reaching the regional lymph nodes, long bones and skull, bone marrow, liver and even the skin, This would manifest with:
Fever, irritability, growth retardation, if it comes to compress the bones would cause bone pain, cytopenias (Decreased blood cells), bluish subcutaneous nodules that are a manifestation of cutaneous metastasis, exophthalmos and periorbital ecchymosis simulating a raccoon eyes appearance

If the disease is localized, it can produce something called mass effect, which occurs due to pressure or crushing of some structure, in some cases it can cause spinal cord compression, intestinal obstruction, and superior vena cava syndrome when compressing it partially or totally. Neuroblastoma can also be associated with a paraneoplastic syndrome called opsoclonus-myoclonus-ataxia syndrome, in which patients present uncontrollable and rapid jerking movements, both bodily and ocular, also affecting coordination and causing cognitive alterations.

Now, let's go to one of the most fundamental and important parts of all medicine, and that is.... Diagnosis, since the different diseases if they are not diagnosed simply are not treated.

DIAGNOSIS


This type of tumor is usually discovered as one or several masses in simple X-rays, CT scans or MRI scans. Although there are also certain tumor markers that would help the diagnosis, and these are the metabolites of catecholamines, which will be in excess, such as homovanillic acid (AHV), and vanillylmandelic acid (AVM), which are elevated in 95% of patients, helping us to confirm the diagnosis.

And we have another alternative, which is the histopathological diagnosis, which is through a biopsy.

TREATMENT


The objective principle of knowing all the disease, is to seek to cure it, that is why the diagnosis is so essential, and depending on this is the treatment, varying according to the type of tumor, the location and its spread, treatment can range from something as complicated as multiple aggressive chemotherapy sessions to surgical removal, or even something as simple as cure itself, as some types may present a spontaneous cure, and all this will depend on the stage in which the tumor is found.

Ahora bien, sea quien sea la persona sin importar en que área nos especialicemos en alguna parte de nuestra vida nos encontramos con palabras como tumor o cáncer, estas simples palabras son suficientes para helarte la sangre, para traer a ti un intenso temor, hoy quiero que conozcan uno de esos innumerables tumores, y es el... **NEUROBLASTOMA**, Uno un tanto desconocido e ignorado, pero que puede llegar a ser más común de lo que nosotros mismos pensamos. Pero, ¿Qué es?
Los Neuroblastomas son Canceres embrionarios del sistema nervioso simpático, es decir que es formado desde el mismo momento de nuestro desarrollo, cuando aun somos fetos desarrollándose en el útero materno, este tipo de tumores pueden variar entre una amplia variedad, pueden ser desde uno con una curación espontánea incluso sin uso del tratamiento, hasta tumores Sumamente agresivos que no responden incluso al más intenso tratamiento, y ciertamente los avances científicos mejoran cada vez mas las medidas terapéuticas aunque aún muchos pacientes sucumben ante las formas agresivas a pesar de todo tipo de tratamientos

¿Qué tipo de personas pueden ser afectadas?

TODO tipo de personas, nadie está exento de haber tenido la probabilidad de sufrir de este cáncer en la niñez, siendo el tumor solido extracraneal Mas Común en los niños, y el Cáncer diagnosticado con más frecuencia en los Lactantes, es decir, el periodo desde el mes de vida hasta el momento en el que cumplen 3 años. En EEUU incluso se diagnostican 600 casos aproximadamente cada año, el 90% de estos se descubren antes de los 6 años, y es ligeramente más frecuente en el sexo masculino.

¿QUE PUEDE LLEVARME A DESARROLLAR EL CÁNCER?

La verdad es que la causa aún en la actualidad en la mayoría de los casos es desconocida, sin embargo, un pequeño número de casos se encuentran asociados a ciertas mutaciones genéticas, además de aumentar el riesgo las exposiciones químicas maternas y hasta paternas.

Este tipo de tumor puede producir metástasis a través de la sangre (hematógena), la linfa (linfática) o por invasión local, llegando a los ganglios regionales, los huesos largos y el cráneo, la médula ósea, el hígado e incluso la piel, Esto se manifestaría con:

Fiebre, irritabilidad, retraso del crecimiento, de llegar a comprimir los huesos provocaría dolor óseo, citopenias (Disminución de las células sanguíneas), nódulos subcutáneos azulados que son una manifestación de metástasis cutánea, exoftalmos y equimosis periorbitaria simulando una apariencia de ojos de mapache.

De estar localizada la enfermedad puede producir algo llamado, efecto de masa, que sucede por la presión o aplastamiento de alguna estructura, en algunos casos puede causar compresión de la médula espinal, obstrucción intestinal, y síndrome de la vena cava superior al comprimirla de forma parcial o total.
El neuroblastoma puede estar asociado también a un síndrome paraneoplásico denominado síndrome opsoclono-mioclono-ataxia, en la cual los pacientes presentan movimientos de sacudidas incontrolables y rápidas, tanto corporales como oculares, afectando también la coordinación y provocando además alteraciones cognitivas.

Ahora bien, vamos a una de las partes más fundamentales e importantes de toda la medicina, y esto es... El Diagnóstico, puesto que las diferentes enfermedades si no se diagnostican simplemente no se tratan.

DIAGNÓSTICO


Este tipo de tumor por lo general se descubren como una o varias masas en radiografías simples, Tomografías o Resonancias magnéticas. Aunque también se cuenta con la existencia de ciertos marcadores tumorales que ayudarían al diagnóstico, y estos son los metabolitos de catecolaminas, las cuales estarán en exceso, como el ácido homovanílico (AHV), y ácido vanilmandélico (AVM), los cuales se encuentran elevados en el 95% de los pacientes, ayudándonos a confirmar el diagnóstico.

Y tenemos otra alternativa, que es el diagnostico histopatológico, este es a través de una biopsia.

TRATAMIENTO

El principio objetivo de saber toda la enfermedad, es buscar curarla, ese es el por qué el diagnóstico es tan imprescindible, y dependiendo de este es el tratamiento, variando conforme al tipo de tumor, la localización y su diseminación, el tratamiento puede ir desde algo tan complicado como múltiples sesiones agresivas de quimioterapia hasta la extirpación quirúrgica, o incluso algo tan sencillo como curarse por sí solos, ya que algunos tipos pueden presentar una curación espontanea, y todo esto dependerá del estadio en el que se encuentre el tumor.
