I recently watched a documentary that talked about the opiate addiction crisis in the United States of America, and how it is slowly consuming society in a very subliminal way. This is something that for me here in Brazil is not very understandable because, perhaps due to well-taken measures, opiates in general are strictly controlled and little prescribed in health, something completely different from what is seen in the USA, where for many years this famous "Big Pharma" fed society with Vicodin, Oxycodone, Morphine and Fentanyl.

But each country has its peculiarities, and if here we are less susceptible to doping with extremely powerful painkillers, this is not the case with benzodiazepines. Although the current medical conscience understands the mistake of prescribing benzodiazepines indiscriminately, we have a complete generation of mothers, fathers and grandparents (not counting teenagers) who use Clonazepam, Alprazolam, Midazolam, Diazepam among others daily, whether for anxiety or insomnia, but this drug class is spread all over the country, in the drawers or closets of the majority of the population. While there is occasional recreational use of opiates (as I could see from Reddit) around here, anxiolytics are in high demand and no doubt also abused.

I myself use clonazepam and I find myself in a kind of internal duel between not wanting to need it to balance some episodes of crisis and finding it the most efficient drug ever made by man, because it has the almost immediate potential to calm and stop a crisis nervous. But at what price? Well, there are thousands of studies claiming (or rather, proving) that a long-term chronic use of moderate and high doses of benzodiazepines causes horrible damage to the user, mainly in cognitive and mnemonic terms. Not to mention the big problem of tolerance and the need to re-dosage as it becomes a habit.

But would we be ready to change the habit of thousands of elderly and adults who use such a drug to sleep? How to manage a silent crisis? And how far is it harmful to worry? As I have already mentioned, currently doctors have avoided indicating benzodiazepines to their patients, and are looking for less drastic and problematic solutions. But it is very difficult to change the long-standing habit of people who are not even interested in this change. But what about young people? Well, as mentioned above, on Reddit I see dozens of teenagers sporting boxes upon boxes of anxiolytics, as well as getting on the bandwagon of taking codeine and promethazine and so on, wrecking their health from an early age. Although things are more subtle here, we are experiencing a kind of silent crisis of these drugs, and calm in a way that will take a long time to negatively affect our society, but what I know is that we will have an army of young people completely destroyed by the use overdose of benzodiazepines and opiates, or in a state of numbness and numbness or in a state of "cold turkey" and sickened by desperate attempts to get out of addiction.

Recentemente assisti um documentário que falava sobre a crise de dependência dos opiáceos nos Estados Unidos da América, e como isso esta consumindo lentamente a sociedade de forma bastante subliminar. Isso é algo que para mim aqui no Brasil não é muito compreensível por que talvez por medidas bem tomadas, os opiáceos em geral são rigorosamente controlados e pouco receitados na saúde, coisa completamente diferente do que é visto nos EUA, onde por muitos anos essa famosa "Big Pharma" alimentou a sociedade com Vicodin, Oxicodona, Morfina e Fentanil.

Mas cada país com suas peculiaridades, e se aqui estamos menos suscetíveis a dopar-se com analgésicos extremamente poderosos, isso não se repete com os benzodiazepínicos. Ainda que a atual consciência médica compreenda o erro de receitar benzodiazepínicos indiscriminadamente, temos uma geração completa de mães, pais e avós (sem contar os adolescentes) que utilizam diariamente Clonazepam, Alprazolam, Midazolam, Diazepam entre outros, seja para ansiedade ou para insônia, mas essa classe medicamentosa está espalhada por todo o país, nas gavetas ou armários da maioria da população. Ainda que exista um uso recreativo ocasional de opiáceos (como pude perceber pelo Reddit) por aqui, os ansiolíticos são muito procurados e sem dúvidas também utilizados com abuso.

Eu mesmo utilizo clonazepam e me vejo numa espécie de duelo interno entre não querer precisar dele para equilibrar alguns episódios de crise e de achá-lo o medicamento mais eficiente já feito pelo homem, por que tem o potencial quase imediato de acalmar e parar uma crise nervosa. Mas à que preço? Bom, existem milhares de estudos afirmando (ou melhor, comprovando) que um uso crônico de longa data de doses moderadas e altas de benzodiazepínicos geram horríveis danos ao usuário, principalmente em termos cognitivos e mnemônicos. Sem falar do grande problema da tolerância e da necessidade de re-dosagem conforme torna-se um hábito.

Mas estaríamos prontos para mudar o hábito de milhares de idosos e adultos que utilizam um medicamento desses para dormir? Como administrar uma crise silenciosa? E até onde é nocivo para se preocupar? Como já comentei, atualmente os médicos tem evitado indicar benzodiazepínicos para seus pacientes, e procuram por soluções menos drásticas e problemáticas. Mas é muito difícil mudar o hábito de longa data de pessoas que nem estão interessados nessa mudança. Mas e os jovens? Bem, como mencionado acima, no Reddit vejo dezenas de adolescentes ostentando caixas e mais caixas de ansiolíticos, além de estarem entrando na onda de tomar codeína e prometazina e assim por diante, destruindo sua saúde desde cedo. Ainda que por aqui as coisas sejam mais sutis, vivemos uma espécie de crise silenciosa dessas drogas, e calma de uma forma que vai levar muito tempo para afetar negativamente nossa sociedade, mas o que sei é que teremos um exército de jovens completamente destruídos pelo uso excessivo de benzodiazepínicos e opiáceos, ou em estado de torpor e amortecimento ou em estado de "cold turkey" e adoentados pelas tentativas desesperadas de sair do vício.
