Introduction to oncology

History

The term cancer (from the Greek "crab" for the grip in which it tightens the tissue of its target) was coined by Hippocrates. "Ónkos" in Greek from which oncology derives and "tumor" in Latin for tumor refers instead to the mass, the swelling that characterizes this disease.
The first description of tumor was made on a papyrus by the Egyptian doctor Imhotep (2500 BC); after that there were few traces of cancer in the ancient medical treatment, because it did not appear to be a problem: in the past life expectancy was much lower than today and usually deaths occurred for different causes (infections, wounds, etc.).

For centuries a halo of mystery surrounded the triggering causes of this pathology until Galen tried to formulate the pathogenesis according to the theory of "bile" of different colors. There were four types, red, white, yellow and black, in particular "black bile" was thought to be the cause of cancer. This theory was only debunked in 1550 by Vesalius, who never found "black bile" during his cadaveric dissections. In 1900, people began to look for the causes of cancer and it was discovered that X-rays, tobacco, viruses and bacteria could be counted among them.

The development of the therapeutic approach to this disease has also undergone major changes. In 1500, for example, breast cancer was removed with pincers, fire and acids. Only in 1700 the approach became more scientific and surgery began to develop (patients, however, still die of pain or infection). The decisive turning point came first with the appearance of anaesthetics and then with antibiotics for the treatment of surgical wounds.
In 1895, people began to think that X-rays, a known cause of cancer, could also be a means of eradicating it. Later on, people began to think about the adjuvant therapy to be carried out after the surgical export, so as to eliminate the neoplastic cells still remaining in situ.

In 1950 the first chemotherapy drugs appeared; the first patient to receive this type of treatment was a child suffering from lymphoma who survived a few more months thanks to the therapy. Afterwards the interaction between pharmacological approach and surgery underwent great changes and variations thanks to many doctors such as Umberto Veronesi.

Epidemiology

Cardiovascular diseases are the main cause of death (37%), however, in second place are cancers (30%). In the USA, however, this percentage has reversed, especially in certain age groups; in fact, thanks to the progression in the treatment of cardiovascular diseases, the main causes of death are tumors. Moreover, cancer is a disease that occurs mainly in old age and the increase in neoplastic pathologies is therefore due to the increase in life expectancy (paediatric cancer is less accidental than cancer in adulthood).

The incidence of the various types of cancer has also changed over time. For example, the incidence of stomach cancer has decreased since 1930, probably due to improved nutritional status. The incidence of uterine cancer has also decreased dramatically, thanks to prevention and early diagnosis. Other cancers, on the other hand, have increased; lung cancer is the emblem, in fact, the curves of increase go hand in hand with the increase in tobacco consumption. In any case, statistics indicate that in Italy the general incidence of cancer is slowly decreasing[ Certain types of cancer such as lung cancer (because the trigger is not eliminated) and pancreatic cancer (because the cause is not yet known) are excluded.

Analyzing the graphs of the survival trend at 5/10 years after diagnosis, it has been seen that there has been an increase in survival for tumors such as prostate or breast cancer, while during the last 30/40 years the mortality of lung cancer has not decreased at all. In addition, the likelihood of dying from cancer is greater in men than in women and as they get older it can become the leading cause of death.

Incidence and mortality are not equivalent, in fact a high incidence may correspond to a low mortality and vice versa (for example prostate cancer has a high incidence but a low mortality). Survival also varies according to the type of tumour; an early diagnosis of prostate or breast cancer allows a cure. On the contrary, lung cancer has a low survival also because early diagnosis is very difficult.

In Italy, there is a gradient of tumor geographical distribution that shows that the incidence of this disease is more evident in the north than in the south (the causes of these different incidences have not yet been clarified). In Lombardy, for example, lung cancer and colorectal cancer have a high incidence. In particular, 23,108 cases of malignant tumours were diagnosed in Brescia in the three-year period 2004-2006. In males the most frequent tumors are prostate tumors, melanomas and lung tumors. In women, on the other hand, breast, colorectal and lung cancers (less important than the male population, but the trend is on the rise).

It has been shown that lung cancer mortality is 97% of the incidence, which means that generally 7 out of 100 patients diagnosed with this disease survive 5 years after diagnosis. The positive note is that the data in Figure 3 shows that the fight against cancer is improving thanks mainly to prevention and early diagnosis, but also through the use of better therapeutic approaches. A great method of prevention for many cancers is the vaccine. Many lives have been saved thanks to the introduction of vaccines against HPV (Papilloma Virus), HBV (Hepatitis B) and many others because of the negligible risk involved. However, these diseases have not been completely eradicated due to new anti-vaccinist trends, which do not allow the right protection and prevention.

Another important concept to underline is that certain graphs show an increase in cancer incidence, but this is also due to the increase in their diagnosis. The deeper the diagnosis in the population, in fact, the greater the probability of finding tumors.

It is also very important to consider the category of people treated. There are, in fact, environments that favour the development of certain pathologies. For example, some mine workers suffer more from lung cancer due to contact with and inhalation of carcinogenic substances.

Globally, there are several cancer incidences, for example, in the Japanese population the incidence of prostate cancer is very low, while in California it is very high. It has been noted, however, that over time the children of Japanese immigrants in California became more ill with prostate cancer. This underlines that although there are genetically determined tumors, there are also many that are due to the environment. Lung cancer is in an intermediate situation where the genetic component (e.g. polymorphism of a certain gene) is an accident, but the fact of consuming cigarette smoke or not considerably increases its incidence and mortality (the probability of developing it can increase as much as 10 times).

Other important factors are diet, followed by physical activity, which when combined with cigarette smoke consumption dramatically worsens the statistics.
We suggest a reflection on the following data: tumours with a higher incidence on the population have as risk factors tobacco use, sedentary lifestyle, wrong diet and alcohol consumption. These are aspects that each of us can control and therefore fall within the field of prevention.

Cenni storici

Il termine cancro (dal greco “granchio” per la morsa in cui stringe i tessuti del proprio bersaglio) venne coniato da Ippocrate. “Ónkos” in greco da cui deriva oncologia e “tumor” in latino per tumore fanno invece riferimento alla massa, al rigonfiamento che caratterizza questa malattia.
La prima descrizione di tumore è stata effettuata su un papiro dal medico egiziano Imhotep (2500 a.C.); dopodiché ci furono poche tracce del cancro nella trattazione medica antica, poiché esso non risultava essere un problema: in passato l’aspettativa di vita era nettamente inferiore rispetto a quella dei giorni nostri e generalmente i decessi avvenivano per cause diverse (infezioni, ferite ecc.).

Per secoli un alone di mistero circondava le cause scatenanti questa patologia finché Galeno cercò di formularne la patogenesi secondo la teoria della “bile” di diversi colori. Ne esistevano di quattro tipologie, la rossa, la bianca, la gialla e la nera, in particolare la “bile nera” era quella che si pensava essere causa del cancro. Questa teoria fu sfatata solo nel 1550 da Vesalio, che nel corso delle proprie dissezioni cadaveriche non trovò mai la “bile nera”. Dal 1900 si iniziarono a cercare le cause scatenanti il cancro e si scoprì che i raggi X, il tabacco, i virus ed i batteri possono essere annoverate tra esse.

Anche lo sviluppo dell’approccio terapeutico nei confronti di questa patologia ha subito grandi cambiamenti. Nel 1500, per esempio, si procedeva asportando il tumore al seno con tenaglie, fuoco e acidi. Solo nel 1700 l’approccio diventa più scientifico e la chirurgia inizia a svilupparsi (i pazienti, però muoiono comunque per dolore o infezioni). La svolta decisiva arriva prima con la comparsa degli anestetici e poi con quella degli antibiotici per la cura delle ferite chirurgiche.
Nel 1895 si inizia a pensare che i raggi X, causa nota di tumori, potessero essere un mezzo anche per estirparlo. Più avanti si inizia a pensare alla terapia adiuvante da effettuare in seguito all’esportazione chirurgica, così da poter eliminare le cellule neoplastiche rimaste ancora in situ.

Nel 1950 compaiono i primi farmaci chemioterapici; il primo paziente a ricevere questo tipo di cure fu un bambino affetto da linfoma che è sopravvissuto qualche mese in più grazie alla terapia. In seguito l’interazione fra approccio farmacologico e chirurgia subisce grandi cambiamenti e variazioni grazie a numerosi medici come Umberto Veronesi.

Epidemiologia

Le malattie cardiovascolari sono la causa principale di morte (37%), tuttavia al secondo posto troviamo i tumori (30%). Negli USA, però, questa percentuale si è ribaltata, soprattutto in determinate fasce d’età; infatti, grazie alla progressione del trattamento delle malattie cardiovascolari le principali cause di morte risultano essere proprio i tumori. La patologia oncologica, inoltre, è una malattia che insorge prevalentemente in età avanzata e l’aumento delle patologie neoplastiche è quindi dovuto all’aumento dell’aspettativa di vita (Il tumore pediatrico risulta essere meno incidente rispetto al tumore in età adulta).

L’incidenza dei vari tipi tumorali è, inoltre, cambiata nel tempo. Per esempio dal 1930 è diminuita l’incidenza del tumore allo stomaco grazie probabilmente al miglioramento dello stato nutrizionale. Anche il tumore all’utero è diminuito drasticamente; questo grazie alla prevenzione e alla diagnosi precoce. Altri tumori, invece, hanno subito un incremento; il tumore al polmone ne è l’emblema, infatti, le curve d’incremento vanno di pari passo con l'aumento del consumo di tabacco. In ogni caso, le statistiche indicano che in Italia l’incidenza generale del cancro sta lentamente diminuendo[ Si escludono determinati tipi di tumore come quello ai polmoni (perché la causa scatenante non viene eliminata) e quello al pancreas (perché non si conosce ancora la causa scatenante).

Analizzando i grafici del trend di sopravvivenza a 5/10 anni dalla diagnosi, si è visto che c’è stato un aumento della sopravvivenza per tumori come quello alla prostata o alla mammella, mentre nel corso degli ultimi 30/40 anni la mortalità del tumore ai polmoni non è assolutamente diminuita. Inoltre, la probabilità di morire di tumore è maggiore negli uomini rispetto alle donne e con il progredire dell’età può diventare la causa maggioritaria di morte.

Incidenza e mortalità non sono equivalenti, a fronte di un’alta incidenza, infatti, può corrispondere una bassa mortalità e viceversa (Ad esempio il tumore alla prostata ha un’alta incidenza ma una bassa mortalità). La sopravvivenza varia anche in funzione del tipo di tumore; un cancro alla prostata o alla mammella diagnosticato precocemente permette una via di guarigione. Al contrario il tumore al polmone ha una sopravvivenza bassa anche perché la diagnosi precoce è molto difficile.

In Italia, esiste un gradiente di distribuzione geografica tumorale che evidenzia come l’incidenza di questa patologia sia più evidente al nord rispetto al sud (Le cause di queste diverse incidenze non sono ancora state chiarite). In Lombardia, per esempio, il tumore al polmone e quello al colon-retto hanno una grossa incidenza. In particolare, nel bresciano nel triennio 2004-2006 sono stati diagnosticati 23.108 casi di tumori maligni. Nei maschi i tumori più frequenti sono i tumori alla prostata, melanomi e tumori ai polmoni. Nelle donne invece, i tumori alla mammella, colon-retto e tumore ai polmoni (dato meno importante rispetto alla popolazione maschile ma comunque il trend è in ascesa).

Si è evidenziato che la mortalità del tumore ai polmoni è pari al 97% dell’incidenza, questo vuol dire che sopravvivono a 5 anni dalla diagnosi generalmente 7 pazienti su 100 a cui è stata diagnosticata questa patologia. La nota positiva è che i dati presenti in figura 3 ci indicano che la lotta contro il cancro sta migliorando grazie soprattutto alla prevenzione ed alla diagnosi precoce, ma anche tramite l’utilizzo di migliori approcci terapeutici. Un grande metodo di prevenzione per molti tumori è il vaccino. Sono state salvate numerose vite grazie all’introduzione dei vaccini contro l’HPV(Papilloma Virus), HBV(epatite B) e numerosi altri a fronte del rischio irrisorio che questa procedura comporta. Queste patologie non sono, però, state del tutto debellate a causa delle nuove tendenze anti-vacciniste, che non permettono la protezione e la prevenzione giusta.

Un altro concetto importante da sottolineare è che certi grafici evidenziano un aumento di incidenza tumorale, ma questo è anche dovuto all’aumento della diagnosi degli stessi. Più si approfondisce la diagnosi nella popolazione, infatti, maggiore è la probabilità di trovare tumori.

Molto importante è anche considerare la categoria di persone trattate. Esistono, infatti, degli ambienti che favoriscono lo sviluppo di determinate patologia. Per esempio certi lavoratori di miniera si ammalano maggiormente di tumori ai polmoni a causa del contatto e dell’inalazione di sostanze cancerogene.

A livello globale vi sono diverse incidenze tumorali per esempio nella popolazione giapponese l’incidenza del tumore alla prostata è molto bassa, mentre in California è molto alta. Si è notato tuttavia che, col passare del tempo i figli di immigrati giapponesi in California si ammalavano maggiormente di neoplasie prostatiche. Questo a sottolineare che nonostante ci siano tumori geneticamente determinati, ne esistono anche molti che sono dovuti all’ambiente. Il tumore ai polmoni è in una situazione intermedia in cui la componente genetica (per esempio il polimorfismo di un determinato gene) è un dato incidente, ma il fatto di consumare fumo di sigaretta o meno aumenta considerevolmente l’incidenza e la mortalità dello stesso (la probabilità di svilupparlo può aumentare anche di 10 volte).

Altri fattori importanti sono la dieta, seguita dall’attività fisica che se associati fra loro ed al consumo di fumo di sigaretta peggiorano drasticamente le statistiche.
Si suggerisce una riflessione in merito ai seguenti dati: i tumori con maggiore incidenza sulla popolazione presentano come fattori di rischio utilizzo di tabacco, vita sedentaria, dieta sbagliata e consumo di bevande alcoliche. Si tratta di aspetti che ognuno di noi può controllare e per questo rientrano nel campo della prevenzione.
