would it be paranoia on my side if I assume that the entire family may be nursing the virus?
I don't think you are being paranoid and looking at the circumstances and how things turned out it would be advisable that they get checkout again and they are found not to be haboring the virus they should proceed to complete the vaccination regime, Hepatitis B is real and shouldn't be joked with

Exactly what I think a rational person should do. If he doesn't care about himself, at least, he should consider the small kids.
