Para que se lleve a cabo el proceso de coagulación de la sangre se requiere de una serie de componentes a nivel sanguíneo. Cuando existe una deficiencia de estos factores en el paciente, es probable que esté en riesgo de presentar complicaciones hemorrágicas es por ello que debe tener en cuenta una serie de cuidados que le permitan disfrutar de una mejor calidad de vida así como a través de un tratamiento específico cuando se presenten situaciones que comprometan su salud.

Factores como la protrombina y la tromboplastina son esenciales, al igual que el factor VIII y el factor de von Willebrand. Una de las coagulopatías más comunes por deficiencia de factor VIII es la hemofilia A, pero en este caso hablaremos de la enfermedad de von Willebrand asociada a la deficiencia de este factor y el papel que juega enfermería en la vigilancia, control de signos y síntomas, así como la administración del tratamiento en dichos pacientes.

Durante esta semana en el servicio de medicina interna pude tratar a un paciente con enfermedad de von Willebrand, se caracteriza por ser un tipo de coagulopatía donde el paciente carece de este factor, lo que aumenta el riesgo de sangrado y puede afectar tanto a hombres como a mujeres. Es básicamente una enfermedad hereditaria que no tiene cura pero que puede tratarse mediante la administración de fármacos.

Una de las principales manifestaciones en este paciente es la anemia severa que se puede evidenciar en laboratorios donde el valor de hemoglobina es de 5 g/dl el valor normal en hombres es de 14 a 16 g/dl, otras manifestaciones que requieren vigilancia es la presencia de hematomas en la piel, sangrado por las fosas nasales y encías, en la mujer es probable la aparición de sangrado abundante durante la menstruación.

El objetivo principal del tratamiento es reducir las complicaciones graves como sangrado en el cerebro o dentro del tubo digestivo, en esta enfermedad se suele administrar factor VIII y von Willebrand suele obtenerse de plasma humano, la dosis es de acuerdo al peso y la condición en que se encuentra el paciente la presentación es en polvo liofilizado de 500 UI que se reconstituye con su propio diluyente. Es importante mantener la esterilidad al preparar el medicamento, debemos utilizar el equipo de transfusión con un filtro especial que contiene dentro del paquete para evitar contaminarlo.

La vía de administración es en bolo intravenoso lento o puede estar indicada a través de una bomba de infusión. Durante el procedimiento debemos verificar si existen efectos adversos como dolor torácico, manifestaciones cutáneas, alteraciones del ritmo cardíaco.
