The extraction of ascitic fluid is known as abdominal paracentesis, it is characterized by the extraction of a serous and light yellowish liquid retained in the peritoneum, a space between the abdominal wall and the internal organs, of course, the characteristics of the color of the liquid may vary. It is a medical procedure carried out by a specialist in internal medicine, although it is true that certain precautions must be taken in addition to knowing if there is any contraindication that prevents it from being performed since it could put the patient's life at risk.

Ascites is the accumulation of fluid in the peritoneal cavity that can originate from various causes, one of which is in patients with liver cirrhosis, portal hypertension, heart failure, tumors, kidney disease, among others. This condition generates abdominal pain and distension, the patient may feel strong abdominal pressure, it also causes shortness of breath, impaired liver function leads to serious complications such as hepatic encephalopathy.

It is important to mention that the extraction of ascitic fluid is performed for diagnostic and therapeutic purposes as a measure to reduce abdominal pressure. In either of these two cases, nursing contributes to the preparation of the patient, it must be indicated before urinating to empty the bladder in order to avoid the risk of injury, the procedure to be performed and the risks involved must be explained, on the other hand, the procedure must be explained. keep the patient at rest, make the peripheral access permeable and record vital signs.

La extracción de líquido ascítico se conoce como paracentesis abdominal, se caracteriza por la extracción de un líquido seroso y de color amarillento claro retenido en el peritoneo, espacio que se encuentra entre la pared abdominal y los órganos internos, por supuesto, las características del color del líquido puede variar. Es un procedimiento médico realizado por un especialista en medicina interna, si bien es cierto que se deben tomar ciertas precauciones además de saber si existe alguna contraindicación que impida realizarlo ya que podría poner en riesgo la vida del paciente.

La ascitis es la acumulación de líquido en la cavidad peritoneal que puede originarse por diversas causas, una de ellas es en pacientes con cirrosis hepática, hipertensión portal, insuficiencia cardíaca, tumores, enfermedad renal, entre otras. Esta condición genera dolor y distensión abdominal, el paciente puede sentir una fuerte presión abdominal, también provoca dificultad para respirar, el deterioro de la función hepática lleva a complicaciones graves como la encefalopatía hepática.

Es importante mencionar que la extracción de líquido ascítico se realiza con fines diagnósticos y terapéuticos como medida para disminuir la presión abdominal. En cualquiera de estos dos casos, enfermería contribuye a la preparación del paciente se debe indicar antes orinar para vaciar la vejiga con el fin de evitar riesgo de lesiones, se debe explicar el procedimiento a realizar y los riesgos involucrados, por otro lado, se debe mantener al paciente en reposo, se debe permeabilizar el acceso periférico y registrar los signos vitales.

Existen contraindicaciones para el paciente como lesión o infección en el sitio de punción, la principal complicación está relacionada con perforación de órganos abdominales, sangrado, hematoma abdominal.

Para el procedimiento debemos preparar los materiales, se enfoca en realizar una técnica estéril con el fin de evitar la entrada de patógenos. El objetivo de enfermería es circular al médico los instrumentos a utilizar, además de monitorear posibles signos de complicación en el paciente, por lo que se debe monitorear el color de piel, signos vitales, patrón respiratorio, de igual manera, posterior a la técnica, controlar el sitio de punción y las características de los apósitos.

También es importante registrar en la hoja de enfermería el procedimiento que se realizó, la cantidad de líquido extraído, las condiciones actuales del paciente junto con la fecha y hora del procedimiento, si hubo alguna eventualidad o no.

Como conclusión podemos decir que es un procedimiento invasivo con poco riesgo de complicaciones, es importante favorecer la técnica estéril para disminuir las complicaciones y permitir una buena evolución, de igual manera a medida que se realiza el procedimiento el paciente puede sentirse mejor debido a que disminuirá la disnea y la presión abdominal, lo que favorece una mejor movilización en la cama del hospital.

Recientemente tuve la oportunidad de realizar este proceder , como parte de las labores que me tocan como médico residente, es importante tener en cuenta que se deben extraer entre 3000 a 3500 ml, aunque teniendo en cuenta la relación riesgo-beneficio y la clínica del paciente se puede extraer un poco más.
