¿QUÉ ES LA APENDICITIS Y POR QUÉ SE PRODUCE?

¿WHAT IS APPENDICITIS AND WHY DOES IT OCCUR?



 Es la inflamación e infección del apéndice vermiforme (se llama vermiforme por su parecido a una lombriz), es una estructura que forma parte de nuestro aparato gastrointestinal; ubicada, generalmente, en la fosa ilíaca derecha.


  Su función sigue siendo un misterio, algunos dicen que es solo un órgano vestigial que quedó de la anatomía de nuestros ancestros, tras las evoluciones. Otros mencionan que tiene al menos 3 funciones: inmunológicas, bacteriológicas y embriológica. Lo que sí es bien sabido es que, es lo que más le da de comer a los cirujanos (chiste jajaja).

  La apendicitis se produce por obstrucción del apéndice vermiforme, esto produce que aumente la presión dentro del órgano, las bacterias que viven dentro de ella empiezan a multiplicarse y a generar mayor cantidad de gases; mientras todo el proceso bacteriano e inflamatorio va sucediendo, el apéndice va pasando por sus diferentes fases hasta culminar en perforación (si no se atiende a tiempo).

  It is the inflammation and infection of the vermiform appendix (it is called vermiform because of its resemblance to a worm), it is a structure that is part of our gastrointestinal tract; located, generally, in the right iliac fossa.


   Its function remains a mystery, some say that it is only a vestigial organ that remained from the anatomy of our ancestors, after evolution. Others mention that it has at least 3 functions: immunological, bacteriological and embryological. What is well known is that it is what most feeds surgeons (joke hahaha).


  Appendicitis is caused by obstruction of the vermiform appendix, this causes the pressure inside the organ to increase, the bacteria that live inside it begin to multiply and generate more gases; While the entire bacterial and inflammatory process is happening, the appendix goes through its different phases until it culminates in perforation (if it is not treated in time).

FASES DE LA APENDICITIS

PHASES OF APPENDICITIS



 Se conocen 4 fases, atravesando cada una hasta que haya resolución de la misma:
  • Edematosa: ocurre cuando la obstrucción se instaura, dura aproximadamente de 4 a 6 horas de evolución. Inicia el proceso inflamatorio y el apéndice se torna congestionada.
  • Supurativa: la multiplicación bacteriana da lugar a la aparición de presencia de pus dentro de la luz del apéndice, dura aproximadamente de 6 a 12 horas. El apéndice se torna más congestionada y con exudado purulento en su superficie.
  • Gangrenosa: El órgano empieza a necrosarse, debido a la falta de riego sanguíneo, dura aproximadamente de 12 a 24 horas. Se torna color rojo oscuro y negro.
  • Perforada: es la fase final, las paredes del apéndice se rompen y se libera todo el contenido hacia el interior de la cavidad abdominal, dando lugar a la peritonitis.


 Nota: En algunas ocasiones ocurre un mecanismo de defensa del organismo, donde, las asas inestinales que están al rededor del apéndice forman como una especie de bloqueo para que no haya infección de la cavidad abdominal, dicho proceso de conoce como PLASTRÓN APENDICULAR. Si el apéndice se rompe dentro de ese bloqueo, se conoce como ABSCESO APENDICULAR. Evitando así la aparición de peritonitis.

  4 phases are known, going through each one until there is resolution of it:
  • Edematous: occurs when the obstruction is established, lasts approximately 4 to 6 hours of evolution. The inflammatory process begins and the appendix becomes congested.
  • Suppurative: Bacterial multiplication gives rise to the appearance of pus within the lumen of the appendix, lasting approximately 6 to 12 hours. The appendix becomes more congested and with purulent exudate on its surface.
  • Gangrenous: The organ begins to become necrotic, due to the lack of blood supply, it lasts approximately 12 to 24 hours. It turns dark red and black.
  • Perforated: is the final phase, the walls of the appendix break and all the contents are released into the abdominal cavity, giving rise to peritonitis.


   Note: Sometimes a defense mechanism of the organism occurs, where the intestinal loops that are around the appendix form a kind of blockage so that there is no infection of the abdominal cavity, this process is known as PLASTRÓN APPENDICULAR. If the appendix ruptures within that blockage, it is known as an APPENDICULAR ABSCESS. Thus avoiding the appearance of peritonitis.

CLÍNICA Y DIAGNÓSTICO

CLINIC AND DIAGNOSIS



  Las manifestaciones clínicas al principio son vagas, inicia como dolor abdominal en la parte alta del abdomen, náuseas, vómitos, falta de apetito, etc. Con el pasar de las horas, el dolor se irradia hacia la fosa íliaca derecha, hay contracción abdominal, aumento de la sensibilidad abdominal.


 El diagnóstico se realiza mediante la exploración física, a través de una serie de puntos dolorosos en el abdomen y signos apendiculares; se debe realizar rayos x simple de abdomen, eco abdominal y/o tomografía, para confirmar el diagnóstico a través de imágenes (en caso de contar con tiempo y recursos).

  The clinical manifestations are vague at first, beginning as abdominal pain in the upper abdomen, nausea, vomiting, lack of appetite, etc. As the hours passed, the pain radiated to the right iliac fossa, there was abdominal contraction, and increased abdominal tenderness.


  Diagnosis is made by physical examination, through a series of tender points in the abdomen and appendicular signs; A simple x-ray of the abdomen, abdominal echo and/or tomography should be performed to confirm the diagnosis through images (if time and resources are available).

TRATAMIENTO

TREATMENT



  • Apendicitis: apendicectomía (extracción del apéndice).
  • Peritonitis localizada: apendicectomía + drenaje + antibióticos.
  • Peritonitis generalizada: apendicectomía + lavado de cavidad abdominal + antibióticos.
  • Plastrón apendicular: antibióticos.
  • Absceso apendicular: drenaje percutáneo + antibióticos.
  • Appendicitis: appendectomy (removal of the appendix).
  • Localized peritonitis: appendectomy + drainage + antibiotics.
  • Generalized peritonitis: appendectomy + lavage of the abdominal cavity + antibiotics.
  • Appendicular plastron: antibiotics.
  • Appendicular abscess: percutaneous drainage + antibiotics.
