The abundant supply of oxygen to the different tissues is of basic importance for organisms which, like humans, have a high metabolic activity. In some lower organisms there is in plasma solution a protein with combinatorial properties with oxygen, Hemocyanin, but this primitive system has been improved in higher organisms, where oxygen is reversibly bound with a protein, Hemoglobin, which is maintained in the blood, within very specialized structures which are the Red Blood Cells or also called Erythrocytes: so specialized that they have lost their nucleus to better fulfill their function of transporting hemoglobin and therefore oxygen.

Human hematopoiesis (Hematopoyesis humana)

Human hematopoiesis: in Wright's stain, May-Grünwald-Giemsa stain or May-Giemsa stain, the morphological characteristics of hematopoietic cells are shown. Alternative names for certain cells are indicated in parentheses; some cells may have more than one characteristic appearance, so more than one representation of the same cell has been included. Morphology (Morfología)

Erythroid cells (immature red blood cells) in the normal adult are derived from undifferentiated primitive medullary cells called hemocytoblasts or "Colony Forming Unit" (CFU), which, under the stimulus of a substance called Erythropoietin, differentiate and transform into the first recognizable cell of the erythroblast line, which is the proerythroblast or pronormoblast. It is worth noting the existence of a normal adult erythroblastic cell line, called normoblastic, as opposed to the megaloblastic line that can be observed, for example, in anemia caused by vitamin B12 deficiency. This is why the term Pronormoblast is used for the first cell identified in the erythroid series, which is presented with the following description:

  • The pronormoblast is a large cell of 14 to 25 µm in diameter with a large nucleus, uniform chromatin distribution, and 1 to 3 well-defined nucleoli; the cytoplasm stains blue with Wright's stain and has a clear pronuclear zone that houses the Golgi apparatus. Electron microscopy shows abundant ribosomes and mitochondria. At this stage there is no identifiable hemoglobin in the cell cytoplasm, although scattered granules of ferritin can be observed.
  • El Pronormoblasto, es una célula grande de 14 a 25 µm de diámetro con un núcleo grande, con distribución uniforme de la cromatina, y de 1 a 3 nucléolos bien definidos; el citoplasma se tiñe de azul con la coloración de Wright y presenta una zona clara pronuclear que aloja el Aparato de Golgi. The next cell is the smallest of the normoblastic series (10 to 15 µm in diameter), and is called Orthochromatic Normoblast, having a small, very dense, pyknotic nucleus, sometimes acquiring various irregular shapes, and pink cytoplasm, practically the same color as the mature erythrocyte. At the end of this stage, the nucleus is lost and the anuclear structure that follows is the reticulocyte, 7 to 10 µm in diameter, containing as its name indicates, a reticulum or chromatic network, which is very well seen with supravital colorations such as cresyl blue.

This network is formed by aggregates of ribosomes, mitochondria and other cellular organs that allow it at this stage of reticulocyte, which lasts 1 or 2 days, to synthesize one-fifth of the hemoglobin contained in the mature erythrocyte. The reticulocyte is very well observed with phase contrast optics.

In these cases, mainly of hemolytic anemia, reticulocytes other than normal may appear. Howell-Jolly bodies are nuclear remnants, the color of the pyknotic nucleus, usually one per cell, caused by incomplete expulsion of the nucleus and may be seen in hemolytic and megaloblastic anemias and splenectomized subjects. Cabor rings are annular or eight-shaped structures, perhaps remnants of the nuclear membrane. After hemolytic crises, cells with iron accumulations in their interior called Siderosomes can be seen, which when observed with the usual stains are called “Pappenheimer bodies”.

Los Anillos de Cabor son estructuras anulares o en forma de ocho, quizás restos de la membrana nuclear. Después de crisis hemolíticas pueden verse células con acúmulos de hierro en su interior llamados Siderosomas, que cuando se observan con las coloraciones habituales se llaman “cuerpos Pappenheimer”.

Within the composition of the erythrocyte, it can be said that it has a very complex structure and the metabolic activities are varied; containing about 60% water and 40% solid matter, from which, it can be deduced that 90% is hemoglobin and 10% stroma. The stroma consists mainly of proteins (40 to 60%) and lipids (10 to 12%).

Erythrocyte Function (Función del Eritrocito)

The main function of the mature erythrocyte is the transport of oxygen by means of its hemoglobin content, and it also acts in the transport of carbon dioxide from the tissues to the lungs for its elimination. When hemoglobin is saturated, it transports 1.34 ml of oxygen per gram and its saturation depends on the oxygen tension of the blood, and this relationship can be studied by means of the oxygen dissociation curve. The characteristics of this curve depend not only on the properties of the hemoglobin molecule but also on a series of variables such as pH, temperature, ionic strength and intra-erythrocyte concentration of phosphorylated derivatives, especially 2,3 diphosphoglycerate (2,3 DPG).

The affinity for oxygen is normally expressed in relation to the oxygen tension where a 50% saturation (P50) occurs, that is, when the hemoglobin is half in the form of oxyhemoglobin and half as deoxyhemoglobin. This value in normal male erythrocytes is 27.1 mm Hg and 27.5 mm Hg in women. If the affinity for oxygen increases, the dissociation curve shifts to the left and P50 decreases, and if it decreases, it shifts to the right and P50 increases.

Due to heme-hem (heme-heme) interactions, where conformational and oxygen affinity changes occur as the different subunits of hemoglobin become saturated, the oxygen dissociation curve adopts a typical sigmoidal shape.

With increasing acidity the affinity of hemoglobin for oxygen is reduced (Bohr Effect), and since in tissues, due to their higher carbon dioxide content the pH drops, the affinity is reduced and oxygen is more easily released, which is then taken up by the tissues.

One of the most effective controls of oxygen affinity is the intra-erythrocyte concentration of 2,3-diphosphoglycerate (2-3 DPG). 2-3 DPG is a metabolite of the erythrocyte glycolytic pathway and combines with hemoglobin in the "deoxy" form decreasing its affinity for oxygen. In other words, the affinity for oxygen is inversely proportional to the amount of 2-3 DPG present in the erythrocyte.

On the contrary, under hypoxic conditions the levels of 2-3 DPG increase, decreasing the affinity of hemoglobin for oxygen and thus facilitating its release to the tissues, which is an important mechanism of adaptation to hypoxia. The mechanisms that regulate erythrocyte 2-3 DPG levels are not known with certainty, although the three enzymes involved in the Rapoport-Luebering short-circuit in the glycolytic pathway, phosphoglycerate kinase, diphosphoglycerate kinase and diphosphoglycerate phosphatase, are important.
