Amigos de esta comunidad hermosa de ciencias, mucho tiempo sin escribir por aquí pero hoy traigo una vez mas mi contenido de odontología, a la consulta odontológica asistió una paciente femenina joven la cual estaba bastante preocupada ya que llevaba varios días con fuerte dolor en la cavidad bucal al rededor de la boca lo cual le estaba imposibilitando de gran manera la alimentación, pero estaba bastante preocupada ya que pensaba que se encontraba en presencia de alguna lesión maligna, lo cierto es que debido a la las caractericas clínicas podiamos observar las ulceraciones presentes en la zona antero-inferior vestibular inferior de los dientes, curiosamente la paciente también presenta exostosis vestibular la cual es un crecimiento de hueso demas que este especialmente suelo producirse de mas en el paladar, piso de boca y en el area vestibular de la cavidad bucal cabe destacar, que es de origen totalmente idiopatico que no se conoce su causa.

Ahora bien la estomatitis aftosa y la exostosis vestibular es bastante común en odontología siendo un motivo de consulta principal, suele se bastante comunes las aftas son conocidas como "sapitos" "llagitas", pero lo cierto es que su verdadero nomre son "aftas", recibiendo por nombre Estomatitis aftosa recurrente cuando el paciente dura más de 15 días con estas y cuando las mismas presentan un tamaño bastante aproximado que incluso lleva a 1cm, generalmente estas tienen un tamaño de 2 mm, pero estas tienen la capacidad de expandirse o producir mas lesiones por toda la cavidad bucal recibe el nombre de "Estomatitis aftosa recurrente", pero ahora bien se preguntaran porque estas suelen originarse y lo cierto es que esta se forman por una pérdida de continuidad en el epitelio bucal, es decir en el mismo tejido de la mucosa produciéndose estas ulceraciones.

Las aftas son la lesiones mas común en la consulta por lo que ya mencione todas la persona en algun momento de la vidas las presentan no tienen predilección, por sexo, edad, puede afectar a todas las personas sin importar por el tipo de raza que se tenga, cabe destacar que la duración que éstas tendrán en la boca dependerá de los factores que la produzca es por eso que es importante conocer cada uno de ellos por lo que haré mención a continuación para que podamos conocerlos, uno de ellos son los factores bucales y locales, como los traumatismos, también alergias a la pasta dental, alguna disfunción de las glándulas salivales, las causas microbianas procesos infecciosos con streptococcus, Stafilococcus mutans entre otras bacterias, más así como la gonorrea, herpes humano y simple, las enfermedades sistemicas: vih, diabetes no controlada, la deficiencia de ácido folico, el hábito de fumas favorece también la aparición de aftas.

La estomatitis aftosa, tiene su tratamiento el cual dependera del tipo de estomatitis, los cuales pueden ser las cremas tópicas y los geles, los unguentos también son eficaces, toda esta presentación se aplica por la lesión, estos ban realizando su acción sobre la ulceración, que componente importante debemos tener en cuenta que deben contener la bencidamina ya que esta es una gran agente antimicrobiana, también debe contener antiflamatoria que ayudará a calmar el dolor, como también la lidocaina que aliviará durmiendo complentamente la zona, estos farmacos tópicos pueden ser el "Gencivol" o "Pinvex" en su presentación comercial. Se debe conocer el factor que produce la estomatitis para colocar el tratamiento idonio, en caso de deficiencia nutricional en paciente debera tomar vitamina B12 y ácido folico, evitar sustancias que produzcan alergias, como tambien el consumo excesivo de tabaco.

Por último es muy importante tener en cuenta que esta paciente también presenta una exostosis vestibular lo que favoreció al roce constante y traumatismo, qué favorecieron las formación, el origen de las aftas en la cavidad bucal, ya que debido a la ubicación que posee esta se encuentra justamente encima de la exostosis vestibular, por lo que la paciente debe considerar en un futuro realizarse la debida remoción de este crecimiento óseo, que posee en este caso individual son producidas debido a lo traumatismo al momento de la técnica del cepillado y masticación.

Finally, it is very important to take into account that this patient also has a vestibular exostosis which favored the constant friction and trauma, which favored the formation, the origin of canker sores in the oral cavity, since due to the location that has this is located just above the vestibular exostosis, so the patient should consider in the future to perform the proper removal of this bone growth, which has in this individual case are produced due to trauma at the time of brushing and chewing technique.
