El tratamiento de conducto, que recibe por nombre endodoncia consiste en la remoción de la pulpa completa del diente cuando la enfermedad de caries dental ha avanzado de manera progresiva, endo quiere decir dentro doncia hace referencia a la palabra diente, cada destacar que este tratamiento tiene variaciones en su terapeutica y esto es debido al cual sea el diagnóstico puesto que si un paciente posee pulpa viva puede estar presente ante una pulpitis irreversible por lo que el tratamiento que se emplea en este caso es la conductoterapia y si un paciente presente necrosis pulpar, es una conductoterapi ya que el diente se encuentra sin vitalidad.

Este martes inicie mi tratamiento de conducto en pulpa necrotica, es decir una conductoterapia en un incisivo central superior izquierdo, pieza dentaria 21. Se debe realizar una adecuada apertura para que las limas al momento de la instrumentación pueda entrar adecuadamente en el conducto, debemos tener ante de iniciar la conductometria, nuestra adecuada longitu tentativa, que es el posible tamaño que debe tener nuestro conducto, una vez que ya nuestra apertura este muy bien hecha, debemos localizar nuestro conducto y este se observará, para esto vamos a utilizar nuestro explorador endodontico que este actuara como dedos para permitirnos localizar la entrada del conducto.

Luego vamos a determinar nuestra longitud de trabajo a traves de la conductometria que este procedimiento es el que nos va permitir instrumental adecuadamente el conducto y no pasarnos de la constricción apical es decir la unión dentina, cemento y conducto al momento de realizar la obturación, puesto que no debemos excedernos de estos limites determinados ya que fracasaria el tratamiento endodontico, se debe tener bien en cuenta nuestra longitud de trabajo para no instrumentar mas allá.

Then we will determine our working length through the conductometry, which is the procedure that will allow us to properly instrument the canal and not go beyond the apical constriction, the junction of dentin, cement and canal at the time of obturation, since we should not exceed these limits determined as it would fail the endodontic treatment, we must take into account our working length to not instrument beyond.

Luego vamos a realizar la preparación biomecanica o la instrumentación del conducto para eliminar todo el tejido infectado en este caso de pulpa necrotica, vamos utilizar la técnica híbrida instrumentando desde el tercio cervical, luego medio y después el tercio apical, de adentro hacia afuera para evitar que estos instrumentos arrastren el téjido infectado hacia la porción apical, por lo que puede originar al paciente, dolor e infección.

Luego vamos a seleccionar nuestro cono principal de gutapercha que debe tener el mismo diametro de nuestra lima memoria, este debe quedar muy bien posicionado, luego comenzamos con la obturación de nuestro conducto con cementos bioceramicos o sealapex, así como también oxido de zinc, luego colcamos nuestra restauración definitiva.
