​Cuando los dientes no tienen suficiente espacio para erupcionar adecuadamente, se posicionan en diferentes formas no estarán adecuadamente alineados por completo, el apiñamiento suele ser muy común porque cabe destacar que algunas personas tienen mayor predisposición al perder dientes temporales de manera temprana, los espacios se van cerrando por lo que los dientes se van posicionando de una manera inadecuada, cuando hablamos de apiñamiento nos referimos a la falta de espacio de la adecuda posición o alineación de los dientes, cuando no se cuenta con suficiente espacio para la correcta alineación de los mismos en las encías.

Cuando existe una discrepancia negativa, mediante el tamaño de cada uno de los dientes y el hueso a esto le llamamos apiñamiento, los dientes presentan medidas en sus diámetros mesiodistales así como también longitud y profundidad, cabe destacar que el grosor que existe en los dientes también la anchura de los mismos podemos denominarlo discrepancia osea, cuando existe una mayor suma de los diámetros mesiodistales y el tamaño de los huesos se producira el apiñamiento dental, también existe una discrepancia positiva el ancho de los dientes sera menor que el tamaño del hueso alveolar, sobra el espacio y aparece el diastema.

El apiñamiento suele generar ciertas consecuencias, al estar los dientes juntos y pegados se dificulta por completo una adecuada técnica de cepillado, por lo que la placa bacteriana no tendra la capacidad de removerse adecuadamente y la misma se va calcificar, favoreciendo por completo la acumulación de los irritantes locales como el calculo, esto puede originar ciertas enfermedades de las encías como la gingivitis y luego avanzar como la periodontitis lo que puede ocasionar con el tiempo la perdida de las estructuras dentarias.

Cabe destacar que debemos saber diferenciar las distintas causas del apiñamiento dental, si este es esquelético u dentales, por lo que puede influir como lo es las alteraciones faciales, tipo facial, diversas alteraciones del crecimiento como crecimiento, respiración y tambien deglución, el apiñamiento puede venir acompañado de alteraciones de numero y también de forma, se ha presentado que en diversos casos existan dientes supernumerarios, así como anomalías de posición como la rotación, gresión, intrusión, extrusión entre otros.

Por último el apiñamiento tiene solución por lo que el mejor tratamiento es la ortodoncia para alinear de la mejor manera nuestros lentes, el ortodoncista debe realizar el respectivo examen de cefalometría para así determinar el crecimiento oseo de los huesos, es decir la relación que estos tienen con los huesos del maxilar superior e inferior, existe tres tipos de apiñamiento el leve, moderado y también el severo, por lo que el especialista determinará como abordar adecuadamente el caso.
