  It's been a while since I don't posted about the current state of my father's battle against a recurrence of his cholangiocharcinoma. It was just his birthday last Saturday and he was happy for sure. He is learning to live while still not cured from this cancer. But we had news that at least make us less negative about it. However, the battle is still long and will take a bit more for him and us to see this disease gone, but probably it is something that will last until his life ends, since he is 71 years old and who knows what happens next.



  After completing 6 months of immunotherapy and chemotherapy, the doctors decided to stop the chemotherapy. The two drugs (gemcitabine and cisplatin) that he was taking are very strong and toxic. The tumor shrank a bit; he had two image exams that showed a good sign that the tumor had reduced. So he will continue with just the immunotherapy for a longer time. He also allowed a more precise exam in his tumor to show the genetic profile to see if there is an alternative, more targeted approach for specific mutations. For all the mutations associated with a type of drug, they only found one mutation at the IDH2 gene, associated with Enasidenib and vorasidenib treatments. However, these drugs had only a good response in patients with this mutation in leukemia. I found a clinical trial with Enasidenib and cholangiocarcinoma, but the results weren't good, and the clinical trial was interrupted since no patient had a promising result.

  Some people still question how it is possible that there isn't a possible cure for a cancer like my father's? It is tricky. I worked with cancer biology for 10 years; if they could find all the solutions, probably this area wouldn't exist anymore. We are dealing with a highly mutagenic group of diseases, and even for the same type of tumors, the genetic profile is different, like my father has a profile with not many options nowadays. But there are many options available for those who fit in some genetic profiles.

  But I think my father has some victories. Pancreas cancer and glioblastoma have the worst survival rates and when you start the treatment, it is usually to late to have some type of good results. I would say that those are hard to find a cure. Other diseases are also challenging to find cures for, especially those that affect the neuronal cells, such as Alzheimer's and ALS. We are far from developing a way to recover these types of cells that usually don't regenerate. Viruses are other trickier targets for drugs. These parasitic organisms are mostly able to perform all needed functions. How to target them without harming us? At least current anti-virals for HIV and Hepatitis can help patients to stay alive even with the virus.

  Cures are challenging for many diseases and I think that we have advanced a lot in the past years with new options. Still far from the final victory in most of the diseases, but making patients like my father have a longer survival is a type of victory. Maybe it is too optimistic that we will find a cure for every single disease that affects humanity? I really think so.

  Faz um tempo que não posto sobre o estado atual da batalha do meu pai contra a recorrência do colangiocarcinoma. Foi aniversário dele no sábado passado e ele estava feliz, com certeza. Ele está aprendendo a viver mesmo sem estar curado desse câncer. Mas tivemos notícias que, pelo menos, nos deixam menos pessimistas. No entanto, a batalha ainda é longa e levará um pouco mais para que ele e nós vejamos essa doença desaparecer, mas provavelmente é algo que durará até o fim da vida dele, já que ele tem 71 anos e quem sabe o que acontecerá a seguir.



  Após completar 6 meses de imunoterapia e quimioterapia, os médicos decidiram interromper a quimioterapia. Os dois medicamentos (gencitabina e cisplatina) que ele estava tomando são muito fortes e tóxicos. O tumor diminuiu um pouco; ele fez dois exames de imagem que mostraram um bom sinal de que o tumor havia diminuído. Portanto, ele continuará apenas com a imunoterapia por mais tempo. Ele também permitiu um exame mais preciso do tumor para mostrar o perfil genético e verificar se há uma abordagem alternativa e mais direcionada para mutações específicas. Para todas as mutações associadas a um tipo de medicamento, eles encontraram apenas uma mutação no gene IDH2, associada aos tratamentos com Enasidenib e vorasidenib. No entanto, esses medicamentos tiveram uma boa resposta apenas em pacientes com essa mutação na leucemia. Encontrei um ensaio clínico com Enasidenib e colangiocarcinoma, mas os resultados não foram bons, e o ensaio clínico foi interrompido porque nenhum paciente apresentou um resultado promissor.

  Algumas pessoas ainda questionam como é possível que não haja cura para um câncer como o do meu pai? É complicado. Trabalhei com biologia do câncer por 10 anos; se eles pudessem encontrar todas as soluções, provavelmente essa área não existiria mais. Estamos lidando com um grupo de doenças altamente mutagênicas e, mesmo para o mesmo tipo de tumor, o perfil genético é diferente, assim como meu pai tem um perfil com poucas opções hoje em dia. Mas há muitas opções disponíveis para aqueles que se enquadram em alguns perfis genéticos.

  Mas acho que meu pai tem algumas vitórias. Câncer de pâncreas e glioblastoma têm as piores taxas de sobrevivência e, quando você inicia o tratamento, geralmente é tarde demais para obter algum tipo de bom resultado. Eu diria que é difícil encontrar cura para esses tipos de câncer. Outras doenças também apresentam dificuldades para encontrar cura, especialmente aquelas que afetam as células neuronais, como Alzheimer e ELA. Estamos longe de desenvolver uma maneira de recuperar esses tipos de células que geralmente não se regeneram. Os vírus são outros alvos mais complicados para medicamentos. Esses organismos parasitas são, em sua maioria, capazes de realizar todas as funções necessárias. Como combatê-los sem nos prejudicar? Pelo menos os antivirais atuais para HIV e hepatite podem ajudar os pacientes a se manterem vivos mesmo com o vírus.

  Curas são desafiadoras para muitas doenças e acho que avançamos muito nos últimos anos com novas opções. Ainda estamos longe da vitória final na maioria das doenças, mas fazer com que pacientes como meu pai tenham uma sobrevida mais longa é uma espécie de vitória. Talvez seja muito otimista pensar que encontraremos uma cura para todas as doenças que afetam a humanidade? Eu realmente acho que sim.

I just can't understand how after billions and billions and researching there's not even 1 single vaccine or definite cure for just 1 of these fucking tumors...

Money can help but it has its limits! We still need to get some advances in technology... I worked a bit with detecting new biomarkers in the plasma in my last minutes in the area, but it is still tricky and noisy data. Interesting that one of the projects was with pancreatic cancer and the other glioblastoma. We found possible interesting things but still need more investigation. The goal is diagnose these type of cancer before having an uncontrolled progression so the fight starts earlier.

Many of my families died from cancer, so I believe there is no cure for it. What we can do is to live with it, because basically they are mutation not some diseases.

That's my conclusion, because I see who survive longer, including my mom, are people who changed their lifestyle and live in strict rules for healthy life

Like anything in life, we try to make the best as possible. Because even when sickness hits us we know that at least we tried to be as healthy person as we could!
