Why 'Morning and Night' Isn't Always Good Enough for Your Medication.
For about two years, I worked as a pharmacy technician. Looking back now, there are so many little things we did that I never questioned. One of them was how casually we handled the timing of medications.
Take Ampiclox, for example. Imagine the prescription says 500 mg every 8 hours. In many pharmacies, it isn't unusual for the pharmacist or technician to simply write "1-1-1" on the carton before handing it over. To us, it meant one capsule three times daily. To the average person, however, it translates to something entirely different: one in the morning, one in the afternoon, and one at night before going to bed.
Those two interpretations are not always the same. The same thing happens with one of Nigeria's most popular antimalarial drugs, artemether-lumefantrine. If you take a moment to read the leaflet inside the pack, you'll notice that the second dose should be taken 8 hours after the first dose. After that, the remaining doses are taken every 12 hours.
But how many people actually receive those instructions? In my experience, many customers simply hear, "Take it morning and night." I never really thought much about it until recently.
For more than three years, I've battled acid reflux. And when I say battled, I mean there were days it became so severe that I genuinely started imagining the worst. Sometimes, my mind wandered as far as the grave. Anyone who has dealt with chronic illness knows that the physical symptoms are only half the battle. The anxiety can be just as exhausting.
Over those years, I tried almost everything. I started with omeprazole and an antacid. No significant improvement. I explored complementary remedies. Still nothing. Eventually, I gave up on finding a cure and settled for managing the condition. I became extremely selective about what I ate. I stopped eating late at night. I adjusted my sleeping position. I learned which foods were likely to trigger an attack.
Those changes helped, but they never completely solved the problem. At some point, acid reflux quietly became part of my identity. Every outing, every meal, every night's sleep revolved around avoiding another episode. Deep down, I knew I should probably go for proper investigations, but fear kept winning. Sometimes ignorance feels psychologically safer than confronting what might be hiding beneath the symptoms.
A few weeks ago, I decided to try omeprazole again. This time, the pharmacist said something I don't remember paying attention to before.
"Take the omeprazole 30 minutes before breakfast. Don't take it together with the antacid."
Then he explained why. Antacids can reduce how well omeprazole works if they are taken at the same time. Leave about two to three hours between them. That tiny piece of information changed everything.
Within days, my symptoms improved dramatically. It honestly felt like magic. Except it wasn't magic. It was timing. The experience made me appreciate that when you take medicine can be almost as important as what medicine you take.
Think about Ampiclox again. Antibiotics work best when the amount of drug in your bloodstream stays above a certain level. That's why the prescription specifies every 8 hours, not simply "three times a day." If you take one capsule at 8 a.m., another at 2 p.m., and the last one at 10 p.m., you've kept fairly consistent drug levels.
But many people interpret "morning, afternoon, and night" as 7 a.m., 1 p.m., and 10 or even 11 p.m. Now you've created gaps of 6 hours, 9 hours, and sometimes 11 hours. During those long gaps, the drug concentration may fall below the level needed to effectively suppress the bacteria.
One of the consequences is that some bacteria survive. The infection may take longer to clear. Symptoms may improve briefly only to return. Even more worrying, repeatedly exposing bacteria to antibiotic levels that are too low encourages the development of antibiotic resistance. The toughest bacteria survive, multiply, and become harder to treat—not just for you, but for everyone else.
The same principle applies to artemether-lumefantrine. The dosing schedule wasn't designed to make life difficult. The second dose is deliberately given 8 hours after the first because that's when drug levels need reinforcing to kill malaria parasites at vulnerable stages of their life cycle. Delaying that second dose until "night time" simply because it's more convenient may reduce how effectively the medicine clears the infection.
The remaining doses then continue every 12 hours to maintain enough medicine in the body until the parasites are eliminated.
That's pharmacology at work.
Drug manufacturers spend years studying not only what dose works, but also when it works best. Those intervals printed on the leaflet aren't random suggestions. They are part of the treatment itself.
Of course, life isn't always perfect. Missing the exact time by a few minutes isn't usually a disaster. But consistently replacing "every 8 hours" with "morning, afternoon, and night," or turning an "8-hour second dose" into "I'll just take it tonight," can reduce the effectiveness of treatment.
I wish someone had emphasized this years ago. Today, whenever I'm given a prescription, I pay far more attention to the timing than I used to. If the instructions aren't clear, I ask. If the leaflet says something different from what I remember, I read it again.

My husband does not understand this. When he is prescribed an antibiotic he becomes casual about the taking it exactly on time. I try to explain why timing is important, but that doesn't always work.
I remember my children's pediatrician told me sometimes to give them a double dose for the first dose and then schedule the remaining doses evenly. Have you every heard that suggestion?
This would depend on the drug. Perhaps there are drugs whose prescriptions can work as indicated. Unless the Ped is also a Pharmacist, I wouldn't take him seriously, though.
I think the same thing goes for paracetamol
In as much as we are trying to reduce it's use it is meant to be prescribed 6hourly and we don't keep to it
