Hoy te quiero contar una experiencia en mi día de guardia, donde tuvimos en una emergencia por un tumor poco perceptible. El día comenzó un poco diferente, porque de costumbre antes de iniciar la jornada tomamos café, y esta vez el café ya se había acabado, aparte los insumos y materiales que dotan diario en el area del quirófano del hospital no habían llegado, se había demorado por parte de la logística, en ese destiempo se perdió 2 horas, tiempo que pudo ser productivo para avanzar en los pacientes que ameritan cirugía.

Today I want to tell you about an experience I had on my day on call, where we had an emergency for a barely perceptible tumor. The day began a little differently, because usually before starting the day we had coffee, and this time the coffee was over, besides the supplies and materials that are provided daily in the operating room area of the hospital had not arrived, it had been delayed by the logistics, in this delay we lost 2 hours, time that could have been productive to advance in patients who require surgery.

Al llegar la paciente al área para evaluarla, se noto que estaba sudorosa, ansiosa, con la tensión baja, con latidos del corazón muy rápidos y fuertes, eran tan fuertes que simplemente al estar frente de ella se podían escuchar, sus familiares estaban preocupados porque no sabían que tenía, nos decían que ella estaba bien, pero en unos días se empezó a poner mal.

When the patient arrived to the area to evaluate her, we noticed that she was sweating, anxious, with low blood pressure, with a very fast and strong heartbeat, so strong that you could hear it just by standing in front of her, her relatives were worried because they did not know what she had, they told us that she was fine, but in a few days she started to get sick.
Se le realiza rápidamente sus exámenes, y se demuestra que la paciente tenía baja la hemoglobina, deshidratada, con dolor abdominal, nos dijo que estaba presentando esos mismos síntomas más debilidad msucular y perdida de peso desde hace 4 meses. Al hacerle un estudio de imagen en el abdomen se vio que había una masa que no era normal. Posiblemente un tumor el cual podía ser el mal de su problema.
Al entrar a cirugía se actúo con mucho cuidado por lo delicado de la paciente. Cuando se logró llegar al problema, nos dimos cuenta que el útero tenía una forma anormal con lesiones que la invadían y adherido en otras zonas de la cavidad abdominal, se tuvo que realizar un procedimiento quirúrgico llamado histerectomía donde se retira al útero a la paciente por prevención de desarrollar un tumor mucho más grande que pondría en grave peligro su vida.
Her tests were performed quickly, and it was shown that the patient had low hemoglobin, dehydrated, with abdominal pain, and she told us that she was presenting these same symptoms plus mucosal weakness and weight loss for 4 months. When she underwent an imaging study of the abdomen, it was found that there was a mass that was not normal. Possibly a tumor which could be the cause of her problem.
Upon entering surgery, we acted very carefully due to the patient's delicate condition. When we managed to get to the problem, we realized that the uterus had an abnormal shape with lesions that invaded and adhered to other areas of the abdominal cavity, we had to perform a surgical procedure called hysterectomy where the uterus was removed from the patient to prevent the development of a much larger tumor that would seriously endanger her health and life.

El diagnostico que se presento fue una fibromatosis uterina, la cual ya venía sucediendo desde hace varios meses atrás y empezó a presentar los síntomas de mas leves a más severos. La operación duro 3 horas aproximadamente más el tiempo de la recuperación posanestésica, pero gracias al equipo quirúrgico y a la buena fe de la paciente la operación fue un éxito, con la paciente bajo observación por su delicado cuadro clínico.
El tumor fue extirpado y fue llevado al departamento de anatomía patológica, donde le harán estudios y conjunto con el cirujano decidirán cual es el mejor tratamiento que tiene que cumplir.
The diagnosis presented was uterine fibromatosis, which had been going on for several months and began to present symptoms from mild to more severe. The operation lasted approximately 3 hours plus post-anesthesia recovery time, but thanks to the surgical team and the good faith of the patient the operation was a success, with the patient under observation due to her delicate clinical condition.
The tumor was removed and was taken to the anatomic pathology department, where studies will be done and together with the surgeon will decide which is the best treatment to follow.

Es muy importante en el caso de las mujeres, cuando están en edad fértil o más de 40 años edad acudir a control con su ginecóloga/o de confianza por lo menos 1 vez al año, y cuando hay antecedentes de familiares de tumores de útero acudir por los menos dos veces al año a partir de los 35 años de edad.
