¡Hola Hiver! Espero estes bien.
Hoy vengo a compartir contigo un caso clínico que pude atender en un día de guardia. Es una técnica poco usual en la práctica de Anestesiología, se trata de la intubación submentoniana o submaxilar.
De manera sencilla de explicar, Se trata de una intubación oral modificada, una vez que pasa el tubo endotraqueal a través de la boca, aborda la glotis y este asegurada la vía áerea del paciente conectado a la máquina de anestesia, se va a derivar (mover) ese mismo tubo a través del piso de la boca por la zona mentoniana (papada / mentón) pasando a través de musculos, venas arterias y nervios.
Esta técnica tiene el objetivo de despejar la cavidad oral y nasal sin ningun objeto que pueda obstruir al momento de la intervención quirúrgica. Indicadas en cirugías nariz, ortognaticá, cráneo, fracturas faciales, patologias orales. Fuente

Today I come to share with you a clinical case that I was able to attend on an on-call day. It is an unusual technique in the practice of Anesthesiology, it is the submental or submaxillary intubation.
In a simple way to explain, it is a modified oral intubation, once the endotracheal tube is passed through the mouth, the glottis is approached and the airway of the patient connected to the Anesthesia machine is secured, the same tube will be bypassed (moved) through the floor of the mouth by the mentonian area (chin) passing through muscles, veins, arteries and nerves.
The purpose of this technique is to clear the oral and nasal cavity of any obstructing objects at the time of surgery. Indicated in nose surgery, orthognathic, skull, facial fractures, oral pathologies. Source

Este caso se trató de un paciente de 58 años de edad quién tuvo un accidente de tránsito automovilístico con diagnóstico de múltiples fracturas en todo el cuerpo, una de las lesiónes más visible fue en la región maxilar (boca). Para corregir esta patología se necesito de un cirujano maxilofacial.
Una vez que ya todo el pabellón quirúrgico estaba listo conjunto con los cirujanos, enfermeras y por supuesto nosotros los Anestesiólogos se inició la intervención.
This case was a 58 year old patient who was involved in a car accident and was diagnosed with multiple fractures throughout the body, one of the most visible injuries was in the maxillary region (mouth). A maxillofacial surgeon was needed to correct this pathology.
Once all the surgical pavilion was ready together with the surgeons, nurses and of course us anesthesiologists, the intervention began.

Previo a evaluar al paciente, la anestesia empleada fue general, dormir totalmente al paciente y realizar una técnica de intubación submandibular. Por no ser una ténica de nuestro uso común fue un poco difícil, pero gracias a todo el equipo médico se pudo ejecutar con éxito dandole un mejor confort al paciente y al cirujano.
Prior to evaluate the patient, the anesthesia used was general anesthesia, put the patient to sleep and perform a technique of submandibular intubation, not being a technique of our common use was a bit difficult, but thanks to the entire medical team could be executed successfully giving a better comfort to the patient and the surgeon.

Esta técnica a pesar de que no es común realizarla tiene poco efectos adversos y complicaciones. Saber la anatomía de lo que se va a hacer es esencial para evitar daños y lesiones postquirúgicas.
